If your MBBS-abroad plan includes eventually practicing in the United States, the USMLE (United States Medical Licensing Examination) — not FMGE or NExT — is the pathway that matters. It's a three-step exam sequence, and the structure has changed meaningfully since Step 2 Clinical Skills was discontinued. Here's how it works for international medical graduates (IMGs) in 2026.

There is no Step 2 CS (Clinical Skills) anymore. It was permanently discontinued in January 2021, and in 2021 NBME/FSMB (the exam's co-sponsors) also formally ended work on relaunching a modified replacement version — Step 2 CS is not coming back in any form. If you see an article, coaching brochure, or forum post describing a "4-step" USMLE that includes Step 2 CS as current, it is describing the outdated structure.
What Replaced Step 2 CS
Clinical and communication skills — previously assessed by Step 2 CS — are now verified through ECFMG Certification Pathways instead of a standalone USMLE step (see below). This is a structural, not cosmetic, change: it moved the assessment out of the USMLE sequence entirely and into the ECFMG certification process.
ECFMG Certification — The IMG Gateway
ECFMG (Educational Commission for Foreign Medical Graduates) certification is mandatory for any IMG who wants to enter US graduate medical education. Without it, you cannot enter the NRMP Main Residency Match or begin an accredited US residency, regardless of your USMLE scores.
To be ECFMG-certified, you generally need to:
- Graduate from a medical school listed in the World Directory of Medical Schools (WDOMS).
- Pass USMLE Step 1 and Step 2 CK (Step 1 must be passed before Step 2 CK results are released as part of certification).
- Complete one of ECFMG's Certification Pathways (Pathways 2–6), which verify clinical and communication skills — the replacement for the discontinued Step 2 CS. Most of these pathways require a satisfactory OET Medicine score for English proficiency, and generally require a medical school graduation date on or after January 1, 2023 for the newer pathway options.
- Complete all of the above within a 7-year window starting from your first passed USMLE exam.
