Working as a Nurse in the UK — Complete Guide for International Nurses (2026)

Gabble Team··4 min read

The UK's NHS is one of the largest employers of nurses in the world — and it actively recruits internationally trained nurses from India, Nigeria, Philippines, and dozens of other countries. This guide covers every step from NMC registration to finding your first NHS role.

Step 1: Assess NMC Eligibility

To register as a nurse in the UK, you need:

  • A nursing qualification equivalent to UK standards
  • 450 hours of clinical practice in the last 3 years (or evidence of equivalent competence)
  • English language proficiency (IELTS or OET)
  • No fitness to practise concerns

Check equivalence: The NMC assesses whether your nursing qualification meets UK standards. The NMC website has specific guidance by country. Many nurses from India, Nigeria, Kenya, Zimbabwe, and Philippines qualify.

Step 2: English Language Requirement (IELTS or OET)

The NMC requires one of the following:

TestNMC Minimum
IELTS AcademicOverall 7.0; each component minimum 7.0
OET (Nursing)Grade B in all four sub-tests
TOEFL iBTOverall 95; Listening 24, Reading 24, Speaking 23, Writing 27

Every single IELTS skill must be 7.0 or above. A 7.5 overall with one skill at 6.5 does not meet the requirement.

Step 3: CBT (Computer-Based Test)

Before coming to the UK, you sit the NMC CBT — a computer-based test of nursing knowledge and competence:

  • 120 multiple-choice questions
  • Topics: clinical nursing, medicines management, ethics, communication
  • Taken at Pearson VUE test centres internationally (including in India, Nigeria, Philippines)
  • Passing: required before you can be considered for UK employer sponsorship

Step 4: Apply to a UK Employer

Once you have:

  • NMC eligibility confirmed
  • English language score meeting NMC standards
  • CBT pass

You can apply to NHS Trusts, private hospitals, or nursing agencies for sponsorship:

Major NHS employers that recruit internationally:

  • NHS England Trusts (hundreds across the country)
  • NHS Scotland
  • NHS Wales
  • BUPA, Spire, Nuffield (private hospitals)

Many employers will arrange Skilled Worker visa sponsorship as part of the recruitment package.

Step 5: Health and Care Worker Visa (Skilled Worker Route)

International nurses qualify for the Health and Care Worker visa — a specific stream of the Skilled Worker visa with:

  • No visa application fee (waived for NHS and social care workers)
  • Faster processing
  • ILR (settlement) eligibility after 5 years
  • NHS surcharge waived (no IHS fee)

Salary requirement (2026): Nursing roles (Band 5) pay approximately £28,407–£34,581 — above the Skilled Worker minimum threshold.

Step 6: OSCE (Objective Structured Clinical Examination)

After arriving in the UK and starting supervised practice with your employer, you sit the OSCE:

  • Clinical skills assessment at an NMC-approved test centre in the UK
  • Tests communication, clinical assessment, and procedure skills
  • Passing the OSCE completes your NMC registration process

Your employer arranges the OSCE — you typically sit it within the first 3 months of supervised practice.

7Advanced nurse practitioner£43,742 – £50,056

IELTS for NMC Registration — Preparation Tips

Most internationally trained nurses find IELTS challenging because:

  • IELTS Writing Task 2 requires academic essay writing (unrelated to clinical work)
  • IELTS Speaking Part 3 requires abstract discussion on social topics
  • IELTS Reading uses general academic texts, not medical literature

Practical preparation approach:

  1. Weeks 1–4: IELTS Writing Task 2 structure — essay format; argument development
  2. Weeks 5–6: IELTS Speaking Part 3 — practise abstract topic discussion daily
  3. Weeks 7–8: Full practice tests under timed conditions

Target: Each skill at least 7.0. Even a single skill below 7.0 means the NMC requirement is not met.
